School system requirement for medication administration must be followed in order for students to take medication during school hours and school sponsored events.

The student’s physician must complete the form outlining procedures required during the school day for the student’s diabetic condition.

Medication Form for Students with Allergic Reactions - To be completed by physician/authorized prescriber.

Prior to a child attending any MSDE licensed, registered or approved child care or nursery school in MD, this form must be completed and submitted to the school where the child will attend.

The Religious Obligation: Request for Absence form must be completed for consideration of a lawful (excused) absence from school under Policy 3000, Religious Observances.
